    £31,163 - £37,875 per annum inc HCAS + £2k Salary incentive

    Are you a nurse who wants to work in a busy specialist area, with a keen interest in Bladder and Bowel? You need to have a can do attitude and experience of working with and assessing patients with bladder and or bowel dysfunction.

    This post offers the chance to develop skills and knowledge within Bladder and Bowel care whilst being supported by team members. We see clients of all ages, many of whom, self refer to the service. The work is varied and you will be working independently within patient's homes and occasionally in a clinic setting, providing assessments and treatments to adults with bladder and or bowel symptoms. There will be a lot of support to achieve the necessary skills and knowledge before you work and assess independently.

    Additional Welcome bonus of £1,000 after 6 months in post /successful completion of probationary period and £1,000 after 12 months in post

    The post holder will undertake a range of delegated tasks, reporting to the Bladder and Bowel Nurse Specialists. You will assist in the provision of a high quality and efficient service to all users. You need to be sensitive to patients' needs and adaptable in working in often a very busy environment.

    Detailed job description and main responsibilities

    To work in collaboration with all Community nursing teams, the acute services, social services, and Allied Health professionals to ensure quality service delivery, using local and national guidelines.
    Support/advise others in the co-ordination of complex Continence cases and liaise with appropriate services in the management of acute "at risk" patients working in support of MDT to facilitate the discharge of patients with bladder/bowel conditions in the community.
    Contribute to the development of the service
    Attend multidisciplinary team meetings and appropriate forums in order to gain and disseminate appropriate information to others in the speciality
    Work collaboratively and in partnership with other practitioners
    Work with Health and Social Care Practitioners within Community, Primary, Social and Acute Care with an interest or involvement in Continence management to develop a comprehensive and integrated Continence service for the patients in Bromley
    Provide training sessions to internal and external staff on continence promotion
    To support the most vulnerable patients by collaborating with practice teams, district nurses, community matrons and other specialist nurses, social and secondary care staff to ensure a high standard of care is provided for these patients.
    Receive and triage direct referrals, to provide assessment of the patient's needs with support of other practitioners
    Develop own knowledge and expertise within the speciality related to patient treatment, care pathways and health promotion
    Assist with the monitoring and review of the effectiveness of interventions with the patient and colleagues, agreeing changes to meet needs and established goals
    Act as a Mentor to students and other members of the TeamsRachel Corney
