We are actively recruiting for two Experienced Housing Specialists to start immediately in 2 hospitals North-West, London. The role will consist of providing support to patients that are ready or nearly medically fit for discharge but are homeless. The role will carry a case load of vulnerable homeless individuals and rough sleepers with complex needs who require additional support and an intensive casework intervention so that they access or accept housing and support in their home area and are linked with appropriate services to sustain the outcome. • Rate: £21.66 PAYE / £27.04 Umbrella p/h • Salary: £40,547.52 per annum • Hours: 36 hours a week minimum Duties will include, but are not limited to: • Meeting Homeless Patients on the hospital wards and building relationships in order to support the individuals • Providing assistance with obtaining relevant documents needed to receive support to access housing / resolve individual homelessness • Assistance with accessing benefits and income needed • Liaisons with appropriate professionals to ensure required support is put in place to access housing to resolve homelessness Requirements/ Knowledge required/desired: • A working experience of working within Housing / Homelessness with working knowledge of upto date legislation • Experience of multi-agency working and establishing partnerships with other professions to achieve excellent outcomes for the service user group • Ability to be self-motivated to work independently and make decisions within a peer setting • Working experience of working with Vulnerable adults with complex needs, preferably within a street-based setting This role is to start as soon as possible and is subject to an Enhanced DBS and 3 years referencing.
