Job Title: Locum Band 5/6 Pharmacist - Acute NHS Ward
Job Description:
As a Locum Pharmacist on an Acute NHS Ward, your role is to provide pharmaceutical expertise and support to ensure safe and effective medication management for patients admitted to the ward. You will work on a temporary or contract basis, filling in for absent or vacant pharmacist positions within the acute care setting. Your primary focus will be on optimizing medication therapy, ensuring appropriate drug use, and collaborating with the healthcare team to deliver high-quality patient care.
Responsibilities:
Medication Review and Management: Conduct thorough medication reviews for patients admitted to the acute ward, considering their medical conditions, allergies, and previous medication history. Assess the appropriateness, efficacy, and safety of prescribed medications. Make recommendations for adjustments, dose modifications, or alternative therapies to optimize patient outcomes.
Medication Ordering and Dispensing: Process medication orders accurately, ensuring that prescribed medications are available and dispensed in a timely manner. Collaborate with pharmacy technicians and support staff to facilitate the medication supply chain, including inventory management, procurement, and medication stock rotation.
Medication Counseling and Education: Provide medication counseling and education to patients and their caregivers, ensuring they understand the purpose, administration instructions, potential side effects, and precautions of their prescribed medications. Address any concerns or questions regarding medication therapy.
Medication Safety and Adverse Event Management: Monitor for potential drug interactions, allergies, and adverse drug reactions. Take appropriate measures to prevent medication errors and promote medication safety practices within the ward. Report and manage any adverse drug events or incidents promptly, following established protocols.
Collaborative Care and Communication: Work closely with the multidisciplinary healthcare team, including physicians, nurses, and other healthcare professionals, to ensure coordinated and comprehensive patient care. Participate in ward rounds, clinical meetings, and interdisciplinary discussions to contribute to treatment planning and decision-making.
Clinical Governance and Quality Improvement: Adhere to relevant clinical guidelines, policies, and standard operating procedures to promote best practices in pharmaceutical care. Contribute to quality improvement initiatives, audits, and medication-related research projects to enhance patient outcomes and safety.
Documentation and Record-Keeping: Maintain accurate and up-to-date medication records, including medication reconciliation, interventions, and clinical recommendations. Adhere to legal and regulatory requirements, ensuring confidentiality and privacy of patient information.
Continuous Professional Development: Stay updated with the latest developments, guidelines, and evidence-based practices in pharmacy and acute care. Engage in continuous professional development activities, such as attending relevant training, conferences, and workshops. Seek opportunities for professional growth and learning.
